We have received Wellness Discount forms from the people who are highlighted in blue. If you (or your spouse) has not turned in a form, you have until Wednesday morning, Dec. 18, 9:00 a.m. to do so. This discount is paid by Dudek & Bock as a way to reward employees who have seen their doctors for a check-up sometime in 2024. The discount is worth $10 per paycheck per employee ($260 total for the year), and $20 per paycheck for an employee & spouse ($520 per year).
You can drop off completed forms in the wire box on the table outside HR. There are also blank forms on the table if you need one.
