Some employees are anxious to get the COVID vaccine; others are more hesitant. If you are one of those wanting to be vaccinated, you are likely frustrated by the process. To be eligible to sign up for a vaccine appointment, you must register. All Dudek & Bock employees should register with the City of Chicago, since we work in the city. Also register with the county where you live. If you have a Loyola doctor, we strongly suggest signing up for a myLoyola account if you don’t already have one. For other hospital systems, you may want to contact your doctor and ask if there is a registration system in place.

If your doctor is affiliated with Loyola Hospital (including Gottlieb or MacNeal), sign up for a myLoyola account at https://myloyola.luhs.org/mychart/.
Please bring documentation to your appointment to show that you are eligible for vaccine as part of a priority group. Examples of acceptable documents include:
- Personal identification (State ID or driver’s license, US or foreign passport, Consular ID, Temporary Visitor Driver License)
- Employee identification (work identification or badge, paycheck stub, CityKey card, letter from employer). A letter and paystub was provided to everyone. Contact Nancy at nhopkins@dudek-bock.com if you have lost yours.
Providers may NOT ask individuals to provide documentation regarding their immigration status.
