Referral Fee for New Employees
Some of our most valuable employees have been referred to Dudek & Bock by a friend or relative. We are now interested in hiring for the following positions: Machine Attendant, Material Handler, Set-up, Tool & Die Maker, and Maintenance Electrician. The company will pay a $400 referral fee to an employee who refers a potential candidate, and that candidate is hired and stays with the company for six months. Current employees are of course welcome to bid on the positions through the union process, but we are also asking if you know of qualified candidates who might be interested in working at Dudek & Bock. Vaccine Update
Some employees are anxious to get the COVID vaccine; others are more hesitant. If you are one of those wanting to be vaccinated, you are likely frustrated by the process. To be eligible to sign up for a vaccine appointment, you must register. All Dudek & Bock employees should register with the City of Chicago, since we work in the city. Also register with the county where you live. If you have a Loyola doctor, we strongly suggest signing up for a myLoyola account if you don’t already have one. For other hospital systems, you may want to contact your doctor and ask if there is a registration system in place.

If your doctor is affiliated with Loyola Hospital (including Gottlieb or MacNeal), sign up for a myLoyola account at https://myloyola.luhs.org/mychart/.
Please bring documentation to your appointment to show that you are eligible for vaccine as part of a priority group. Examples of acceptable documents include:
- Personal identification (State ID or driver’s license, US or foreign passport, Consular ID, Temporary Visitor Driver License)
- Employee identification (work identification or badge, paycheck stub, CityKey card, letter from employer). A letter and paystub was provided to everyone. Contact Nancy at nhopkins@dudek-bock.com if you have lost yours.
Providers may NOT ask individuals to provide documentation regarding their immigration status.

Important Changes to FSA
Due to COVID legislation, we have requested changes to the company’s FSA plan. The changes will extend the Grace Period to 12/31/2021 (used to be 3/31/2021), allow members to change their 2021 elections (within some constraints), allow EEs that pulled out of the FSA this year to spend down their account in 2021.

Update on COVID Vaccinations in Cook County
Cook County is currently in Phase 1B of the State of Illinois Vaccination Plan, which includes manufacturing employees. The vaccine supply is limited, and it has been difficult to get an appointment. To register to be notified of vaccine availability, please go to https://vaccine.cookcountyil.gov/. When you go for a vaccine, you will need to provide proof of your priority group eligibility. Letters with pay statements were recently distributed to our employees.
