You can receive a discount on medical insurance if you (and your spouse if he/she is under our insurance) have been vaccinated against COVID-19. That is the only criterion for the wellness discount this year. The discount is $10 per pay period for the employee and $10 for the spouse: a annual total of $260 for employee only or $520 for both.
If you have not been vaccinated, you have until Friday, December 10, to provide HR with your vaccination card. If this is the first of a 2-dose vaccine, we will give you the wellness discount with the expectation that you will return in January with the card showing that you are fully vaccinated. If you do not become fully vaccinated, the wellness discount will be removed.
Most of our employees have been vaccinated (all but 21). If you want the discount for your husband or wife you have until December 10 to bring the vaccination card to HR.
Open Enrollment for 2022 benefits officially begins on Monday, December 6. On Monday benefit books and enrollment forms will be distributed by supervisors. Everyone will get the book in English. If you want a copy in Spanish, Polish or Vietnamese, the translations will be available on a table outside the HR office, or you can view them on the Dudek & Bock website.
EVERYONE will need to fill out, and sign, the enrollment form, even if you do not want medical, dental, vision, or voluntary life. If you do not plan to take D&B’s medical insurance you need to sign a waiver. The waiver forms will also be available on the table outside HR. There is also a box there to drop off completed forms.
If you plan to enroll in the HMO, you will need to look up the PCP (Primary Care Physician) number for you and other family members on the UHC website. Instructions are here: The physician name & PCP# is required on the enrollment form.
(Because we are changing carriers, and due to a switch from Maxwell Health to ADP for benefit tracking, a self-service option is not available this year.)

Dudek & Bock will be open on November 22, 23, and 24 (the days before Thanksgiving). Thanksgiving Day and the Friday after Thanksgiving are paid holidays. Normally, you would be required to work 4 hours on the day before the holiday to be paid for the holiday. This rule is being waived for Nov. 24, but you MUST let your supervisor in advance know if you plan to take off.
